Daniel, have tried to follow the various post on this problem.
Am using Version 1.6.1 of Cover-Wp, thumbs are in uploads folder, permissions have been set, and am getting the frame for the thumb, but a broken link.
Did I miss something?

HubDaniel TaraKeymasterBy default the script works only with images hosted on the same domain. If they’re hosted on a different domain, you need to edit the file and add it to the domain whitelist.

Daniel TaraKeymasterOh, so you’re not manually defining them. Does it generate an image tag? How does the URL to the image look like?
Try manually defining them on the set featured image link.
